Flex-a-lite has become aware of a possible contamination that may have been introduced to their 400 series fans during the molding process. Flex-a-lite STRONGLY RECOMMENDS that you examine your fan to identify the production date. If product was produced in 4/09 we will replace your fan FREE of  charge.

Products affected were produced in April of 2009. You can identify the date of production by looking at the date wheel that is molded into the hub portion of the fan on the engine side. The size of the date wheel will be smaller than a dime, and there will be an arrow pointing to a number. If this arrow is pointing to the number "4", then check the numbers on either side of the arrow. If these numbers are a "0" on the left side and a "9" on the right side, then this is a product that must be replaced.
